Withdrawal from a Course and Leave of Absence

  1. Leave of Absence: A student who must suspend her/his studies at Loyola is advised to fill out a Leave of Absence form if the student plans to return to Loyola. Leave of Absence forms may be obtained from the School of Nursing Office or the Office of Student Records. The leave of absence is good for one year and allows the student to return to Loyola without reapplying for admission.
  2. Withdrawal from Courses: After the drop/add period and up to one week following the mailing of the fall or spring midterm grade reports, students may receive an administrative withdrawal from a course. A grade of W for the course is placed on the transcript. The student is required to submit a course withdrawal form with the adviser's and instructor's signatures. Course withdrawal is not complete or official until all signatures have been obtained and a copy is filed in the Office of Student Records. Course withdrawal forms may be obtained from the Office for Professional Studies, the School of Nursing Office or the Office of Student Records.
